Status Of Access To Eye Health Care In 5 Countries In Sub-Saharan Africa At Pierre Fabre Foundation


Context

Presentation of the Pierre Fabre Foundation

The Pierre Fabre Foundation (hereinafter the “ FPF ”), recognized as a Public Utility in 1999, aims to enable the most disadvantaged populations to access quality care and medicines. In 2023, the FPF is leading 35 programs in 20 countries on five areas of intervention including dermatology which aims, on the one hand, to promote access to dermatological care through tele-expertise and, on the other hand, to improve holistic management of the needs of people with albinism (hereinafter “ PAA ”) through prevention, training and medical care programs, as well as the provision of educational scholarships and socio-strengthening economic.

These interventions are part of a dual approach: support with scientific and medical expertise and sustainability via a partnership approach with local authorities and stakeholders. Since 2015, the FPF has become a recognized player on the theme of albinism both within the local associative fabric of PAAs on the African continent and among political authorities and international bodies which regularly request its contribution. The FPF and its local partners run support programs for around 10,000 PAAs in Mali, Tanzania, Malawi, Togo, Uganda and Ivory Coast.

Presentation of the APPASAAS project

Albinism and consideration of disability are largely under-addressed in the public policies of the countries concerned. Dedicated care pathways adapted to the specific needs of PAAs are not defined and/or poorly available and professionals (health or educational personnel) are often poorly trained in these specificities. Although PAA associations exist across the African continent, they are mostly unstructured and their actions fragmented. They have few means to make their voice heard and participate in decision-making processes that concern them. For questions of access, sustainability and equity, it is therefore essential to support the associative fabric towards coordination and operational autonomy, so that PAA organizations can play their role, particularly in terms of information and awareness.

The project, through a first phase of three years, aims to strengthen protection, prevention and access to care for PAAs in Ivory Coast, Uganda, Niger, DRC and Madagascar (hereinafter “ Project “).

The Project will seek to complement the programs already undertaken with its own funds by the FPF in some of these countries and intended to achieve the following three convergent results:

  • Strengthen the capacities of PAA associations by supporting them in their structuring and networking (1);
  • Improve orientation towards holistic management of AAP (2);
  • Contribute to advocacy for the establishment of national policies for the protection of PAAs (3).

Diagnostic context

The aim of eye care services is to reduce avoidable visual impairment, which is a global public health problem, and to ensure that visually impaired people have access to rehabilitation services. This involves setting up a system to ensure the continuity of quality eye care, comprehensive and integrated into the health care system in the broad sense, at the primary, secondary and tertiary levels while respecting equity and gender, as well as the needs of vulnerable, marginalized groups and communities poorly covered by its services. In order to move towards universal access to quality, comprehensive and integrated eye care it is necessary to evaluate existing services based on factual data on the causes of visual impairment in the population and the gaps in the existing eye care offer.

The World Health Organization (hereinafter “ WHO ”) has developed and is making available an eye care services assessment tool to collect data and information on provision at national and district levels. . Section 1) covers the availability of data on the prevalence and causes of visual impairment, gaps in eye care services, and the use of evidence to obtain necessary support for the eye health sector. Section 2) presents guidance for assessing the status and functioning of a country’s eye care services, based on the six domains of the WHO framework for health systems strengthening. Section 3) provides guidance for determining the place given to eye health in multisectoral programs and for establishing national and international partnerships in the field of eye health. The diagnosis to carry out an inventory of eye health services in the countries targeted by the FPF will be based on WHO recommendations and tools (hereinafter the “ Diagnosis ”).

Also, the FPF wishes to benefit from the services of a Consultant in order to carry out a diagnosis of the care offer and the accessibility of eye health care in each of the following five (5) countries: Ivory Coast, Madagascar , Niger Democratic Republic of Congo, Uganda. All tasks will be entrusted to the Consultant.

Diagnostic objectives

The Diagnostic will focus on:

  • The national strategy of the Ministry of Health and more precisely of the national eye health program and in connection with the recommendations of the WHO and the SDGs [1] which aim for access to quality ophthalmological care for everyone and integrated into all levels of the health system;
  • An inventory of the actors, CSOs / NGOs [2] and private services (clinics and private practices) in eye care will complete the report in order to have a broad vision of the range of actors in eye health;
  • Information relating to trained and available human resources, the offer of services, the accessibility of care as well as the quality of care and monitoring of patients in order to have an overall vision of the situation;
  • Identify existing training courses in the country, identify training curricula and the number of health professionals trained by socio-professional category;
  • A map of the actors.

The tools for conducting the Diagnostic are those recommended by the WHO for the collection of data and information, a literary review will centralize the key reference documents and a mapping of services will be carried out. Documentary reviews and interviews with a wide range of health stakeholders and field visits are to be expected.

All of the activities referred to above to be carried out by the Consultant for carrying out the Diagnostic are hereinafter referred to as the “ Consultative Services ”.

Expected results

The information and data collected during the Advisory Services as well as the analysis results will be presented in an analytical Final Report (as defined below) as well as a country report in order to provide a clear overview of the situation. the supply and accessibility of eye health care services for each of the five (5) countries. A feedback session after the completion of the Advisory Services in each country will be organized as well as a final feedback after submission of the Final Report.

Schedule of Advisory Services and Deliverables

Calendar

The Advisory Services will be carried out over a period of 5 to 10 days for each of the five (5) targeted countries. It will be up to the bidder to propose its own schedule, taking into account the start dates of the Advisory Services as well as the final validation of the Final Report.

  • Within fifteen days following the signing of the Consultative Services contract, a scoping meeting bringing together the Program Manager of the Pierre Fabre Foundation, the Project Coordinator and the Consultant will be organized. The meeting may be held remotely. The main objective of this meeting is a good understanding of the Project by the Consultant, the scope of the Evaluation and the expected results.
  • One month after the completion of the Advisory Services in each of the five countries, the Consultant will send to the Program Manager of the Pierre Fabre Foundation and the Project Coordinator a Diagnostic report for the country concerned (the “ Country Report ”). In the week following this sending and respecting a reasonable deadline for review of each draft Country Report by the Program Manager of the Pierre Fabre Foundation, the Consultant will organize a meeting with the Program Manager and the Project Coordinator in order to discuss this Country Report to review the latter’s comments.
  • Halfway through the implementation of the Advisory Services, the Consultant will organize a meeting with the Program Manager of the Pierre Fabre Foundation and the Project Coordinator in order to communicate to them the first results of the Diagnostic, make any proposals and recommendations that he deems relevant. in view of the expected results of the Diagnostic.
  • No later than fifteen (15) days before the end date of the Advisory Services, the Consultant will send the draft Final Report to the Program Manager of the Pierre Fabre Foundation. In the week following this sending and respecting a reasonable deadline for review of the draft report by the Program Manager of the Pierre Fabre Foundation, the Consultant will organize a meeting with the Program Manager and the Project Coordinator in order to discuss this draft report. report and review its comments.
  • After this meeting, the Consultant will communicate the final version of the Final Report incorporating the comments and remarks of the Pierre Fabre Foundation (the “ Final Report ”) and will organize a meeting to review its evaluation in which the Program Manager of the Pierre Fabre Foundation will participate. , the Project Coordinator and the Consultant. In accordance with the timetable defined below by the Pierre Fabre Foundation, the final version of the Final Report must be communicated to the Pierre Fabre Foundation no later than April 14, 2025.

Deliverables

The Consultant undertakes to submit to the Pierre Fabre Foundation (i) a Country Report for each of the five countries – Ivory Coast, Madagascar, Niger, Republic of Congo, Uganda, then (ii) draft Final Report and a Report Final final no later than April 14, 2025.

It is expected in the Final Report:

  • A summary page;
  • The methodology used (weighting criteria, documentary analysis, interviews carried out)
  • A map of existing services;
  • An inventory of service offerings, HR capacities, accessibility and quality of care;
  • An analysis of the situation;
  • Recommendations.

Technical and financial proposal

The Consultant invited to tender must provide the following elements to the FPF:

  • A technical proposal which must in particular indicate the proposed methodology and the work plan;
  • The provisional intervention schedule respecting in particular the deadline for submission of the Final Report;
  • The detailed financial offer (specify, for each expense, the eligibility conditions and the content of the price: the maximum amount per hotel night if refundable, the class of air or rail transport and its maximum amount if refundable, inclusion or not in the price of the per diem local transport costs, communications costs, etc.);

The financial offer must in particular indicate the total costs of the Consultative Services excluding VAT and including VAT in euros and the payment terms (schedule).

It is specified that the price of the Consultative Services is global and fixed and that the maximum budget for these Consultative Services is 50,000 euros including tax and must include all of the Consultant’s costs and profits as well as any tax obligation for which he may be liable. .

Required profile

The PFP wishes to entrust the carrying out of the Diagnostic and related Advisory Services to a Consultant with the following skills and abilities:

  • Expertise and experience in the field of eye health;
  • Expertise and experience in organizational diagnosis;
  • Expertise in the field of project engineering, strategic planning;
  • Proven expertise on gender issues;
  • Dialogue and listening skills.

Safety

By bidding, the Consultant agrees to strictly comply with the safety rules in force in the countries where it operates. It is the Consultant’s own responsibility to scrupulously follow local and national safety directives. If in doubt, the Consultant must seek clarification from their designated manager. Any breach of these rules could result in sanctions or even termination of the Contract.
