MAIN PURPOSE

Organize and perform anesthesia operations in accordance with the universal rules, protocols and hygiene standards of MSF, in close collaboration with surgeons and medical staff, and strengthen the knowledge and practical skills of anaesthetist staff, through on-the-job training and knowledge transfer, and, to ensure the quality of patient care. Ensure the support of nurse anesthesiologists in the management of post-anesthetic complications and the implementation of the general principles of anesthesia and resuscitation

  • Provide pre-operative consultations to assess the operational risks in collaboration with the nurse anesthesiologist, the surgeon, provide appropriate information to the patient and decide on the type of anesthetic to be used, in order to guarantee optimal conditions prior to the intervention. Proceed with all surgical anesthesias safely and appropriately.
  • Ensure proper preparation of anaesthetics, equipment and equipment required in advance and train teams of nurse anaesthetists on the efficient management of medicines and equipment.
  • Administer anesthetics and maintain anesthesia in accordance with the rules of hygiene and asepsis in force. Also ensure that universal aseptic precautions are maintained in the operating room and PCI measures are in place.
  • Accompany and monitor patient parameters throughout the procedure and ensure that nurse anaesthetists are adequately equipped to manage any complications per and post-operative related to anaesthesia.
  • With teams of nurse anesthesiologists and other members working in the operating theatre, ensure a good transfer of patients to the recovery room and their return to a state of complete consciousness, and define the analgesic protocol.
  • Ensure that basic measures in general resuscitation are in place for critical patients and that related training is organized for doctors and other members for a good continuity of care.
  • In collaboration with the surgeon, advise appropriate post-operative care, including intensive care, including fluid management, oxygen monitoring, airway management, etc, and physiotherapy and torso mobilization, with special attention to pain relief during the awakening process.
  • Implement and supervise hygiene rules and procedures as well as MSF protocols in the operating and recovery rooms and concerning anaesthesia equipment (decontamination, sterilisation, storage conditions, etc.) to ensure the safety and protection of patients and staff.
  • Accompany the nurse anesthesiologists of the operating room in the supervision of the pharmacy of the operating room (orders, inventories, etc, expired products) and ensure the maintenance of the operating room registry and the monitoring of the consumption and management of empty bottles of narcotics, in order to maintain an adequate stock.
  • Ensure the proper functioning and maintenance of the equipment in the operating room (oxygen extractor, ventilation, etc.).
  • Organize the tasks of the staff under his responsibility and participate in other team management activities (recruitment, training, performance evaluation, motivation, internal/external communication, etc.) to ensure appropriate resources (staff, skills and abilities).
  • Collaborate with other project departments (responding to emergency requests, intervening to support other services including emergencies, intensive care, post-operative, etc, post partum and delivery rooms, etc.) and participate in any emergency activity or exploration mission to globally support the MSF project with its expertise and skills.
  • Ensure compliance with administrative procedures (complete patient records, forms, statistics, databases, etc.) and prepare regular reports covering the results, including, procedures and problematic situations and cases, in order to have correct and up-to-date information about the daily activity and to support decision-making.
  1. Responsibilities Specific to the MSF Section / Context
  • Identify the technical difficulties related to the practice of’anaesthesia and the management of patients in postoperative with the aim of’ to provide solutions to reduce morbidity and mortality.
  • Remind nurse anesthetists about the different protocols and possible alternatives.
  • Follow the respect and the’ application of the different protocols to the operating room, the recovery room and the maternity hospital.
  • S’ ensure that resuscitation materials are available in intensive care units (CRENI, PEDIATRICS,NEONATOLOGE and MATERNITE).
  • S’ensure that reanaimation drugs are available in each’ emergency kit in the different departments.
  • The care, medical care and cross-sectional follow-up of patients hospitalized in intensive care, in compliance with MSF protocols.
  • Participates in the multi-quotian visit of patients in intensive care (pediatrics, creni, neonatology and maternity).
  • Management of unstable or serious patients in the emergency room or in other services.
  • Work in close collaboration with emergency departments, operating room and hospitalization services.
  • Provide training on cardiovascular resuscitation, shock, rehydration in a newborn, child and adult.
  • Technical support of resuscitation to teams
  • Follow the sterilizers for the proper constitution of the reaniamtion kit
  • Constitute a complete resuscitation kit for emergencies and intensive care (pediatrics and maternity)
  • Implement a resuscitation protocol (heart failure, renal failure, hypovolemic shock, etc.).
  • Proposal and recommendation in its specific areas of competence: antibiotic therapy, hemodynamics ….
  • In collaboration with the various activity managers and in his field of competence, he / it provides supervision and ongoing training of the team (medical and para-medical) practicing in intentional care, emergencies, recovery room or hospitalization. For this, it relies on protocols and educational resources validated by MSF.
  • Accompany the person responsible for the prevention and control of’infection and hygienists of the operating room in the’work organization while ensuring that universal precautions are always respected and contribute to reducing biological risks and’improvement of infection control with a particular focus on prevention measures against covid-19.
  • Work in collaboration with MSF and MoH doctors in the various departments to maintain the climate of collaboration and ensure the transfer of competence gradually.
  • Organize and participate in the analyses of deaths of all services and S’sensure that’a plan of action and follow-up of recommendations is in place.
  • Discuss with the medical profession the directions to be taken in the management of complicated cases as well as any obstacle that may impact the delivery of quality care.
  • Inform your direct supervisor or the MAM/Gynecologists in time of any problem that may hinder the conduct of activities at the’hospital.
  1. In collaboration with NAM/MAM, S’ ensures the quality of storage and availability of medicines at block op. while setting up a monitoring and control system.

Conditions Required

  • Essential – Diploma of anesthesiologist-reanimator recognized by the State.
  • Essential – Level B2 French. English B2.
  • Two years of experience as an anesthesiologist-reanimator Physician.
  • Wanted – Experience working with MSF or other NGOs in developing countries.
  • Wanted – Experience in the Sahel.
  • Essential – Mastery of office tools (Word, Excel, Power point, Internet..).
